I consent to the processing of my personal data by Enlight Michał Dąbal and Wit Dąbal SC based in Warsaw, 02-964, ul. European 32B (hereinafter: "Enlight SC") in order to answer the inquiry sent via the contact form, which data are provided to Enlight SC by me personally - in accordance with the Act of May 10, 2018 on the protection of personal data (Journal U. of 2018, item 1000) and in accordance with the General Regulation on the Protection of Personal Data (hereinafter: "GDPR") *,
in the following scope: name and surname, e-mail address.

I declare that I have been informed that:
1. The administrator of personal data is Enlight SC based in Warsaw, 02-964, ul. European 32B, email address: info@enlight-led.pl
2. Personal data provided in the contact form will be processed for the purpose covered by the consent, ie in order to answer the question sent, pursuant to art. 6 section 1 lit. a) GDPR.
3. My personal data will be processed for a period of 24 months from the date of consent, and after that time will be deleted, unless the consent granted is withdrawn earlier by me.
4. The recipients of my personal data are entities cooperating with Enlight SC in the handling of mailboxes and other entities authorized to receive personal data on the basis of legal provisions.
5. I have the right to access my personal data, rectify it, delete it, limit data processing, the right to transfer data, the right to object to the processing and the right to withdraw consent at any time, but this does not affect the earlier lawfulness of processing data.
6. I have the right to lodge a complaint with the supervisory body (the President of the Personal Data Protection Office) if I find that the processing of personal data violates the provisions of the GDPR.
7. Providing my data to Enlight SC is voluntary, but necessary to achieve the purpose referred to in point 2.
